Our TCM program offers our clients personalized support to help navigate life’s challenges. Here are just a few ways we can assist:

Transportation Assistance
Need help getting where you need to go? Our Case Managers will guide you through options like Medicaid Taxi services or help you learn to navigate local bus routes, making sure you can access essential appointments and resources. -
Food and Nutrition Support
Struggling to put food on the table? Our team can assist with applying for food stamps or connect you with local food banks like Manna, ensuring you and your family have access to nutritious meals. -
Housing and Educational Guidance
Ready to take the next step toward stability? Whether it’s learning how to use a computer, applying for housing programs through HUD or HOC, or exploring other educational opportunities, we’re here to support you every step of the way.
Who is eligible for TCM services?
To qualify for TCM, clients must have:

A mental health diagnosis

One of the following additional needs:

Support to prevent inpatient psychiatric treatment.

Assistance to avoid homelessness.

Guidance to prevent incarceration or support following release from a detention center or prison.

Support to maintain community-based programs

Who can benefit from TCM?
You may benefit from our TCM program if you:

Are not currently connected with medical services.

Need help with basic necessities such as shelter, food, or income.

Are transitioning from one level of care to another.

Referrals & Eligibility
Interested clients can complete our referral form and submit it with a copy of your ID and your insurance card. We accept Maryland Medicaid, Medicare, and self-pay clients. Vesta accepts uninsured clients in the state of Maryland.
